OISVS Travel Signature Request Form
If you are a current ACU student making a short trip outside the U.S., complete this form to request a travel signature on your I-20 or DS-2019 to facilitate your reentry into the U.S.
Don't forget to drop off your document in the OISVS at 124 Admin at least 3 business days before your departure from Abilene, unless you request a digital form.
