This form is for students who need to request an extension to their document end date to complete their program of study. You will need to upload your most recent I-94, your valid passport ID page, financial documents showing funding for the period of the extension, and a medical letter if applicable. You must submit your new extended I-20 to the Human Resources department if I am working on-campus.
