Use this form to begin your request for work authorization due to unexpected economic hardship by agreeing to your responsibilities and requesting a recommendation from the Office of International Students and Visiting Scholars. You will need to upload your cover letter and documentation, your most recent I-94 and your valid passport ID page.

Should the OISVS and USCIS approve my request for work authorization due to unexpected financial hardship, I understand that I am responsible to maintain my F-1 status by following all applicable laws, including (but not limited to):

Report to OISVS within 10 calendar days any change in my residential address, phone number or major field of study.

Not to exceed the employment limits (20 hrs during the Fall/Spring, full-time in Summer between semesters)

Keep my passport valid at least six months into the future

Comply with U.S. tax and Department of Labor laws

I acknowledge that:

My hardship authorization will end when I transfer to another university, complete my program or violate my status in any way and I must stop working at that time.
